Journey’s Memory Care program provides a secure, structured environment where residents receive personalized support from a dedicated Care Team trained in the complexities of memory loss. Our physicians, nurses, caregivers, and therapists work together to provide therapeutic activities, cognitive exercises, and compassionate guidance that help reduce anxiety and promote a sense of belonging.

Memory Care provides structured, specialized support for individuals with Alzheimer’s disease, dementia, or cognitive impairment. It includes 24/7 supervision, therapeutic programming, and a secure environment, as defined by the National Institute of Health (NIH)
Individuals who need skilled care, additional supervision, support with daily tasks, or a secure setting due to memory loss, cognitive decline, or diseases like Alzheimer’s will benefit from Memory Care.
Memory Care includes enhanced safety features, specialized programming, and Care Teams with additional dementia training. Long-Term Care provides foundational skilled nursing and customized care, including therapy, nutrition, activities, transportation, and more.
Journey uses criteria-based assessments to build individualized care plans, meal plans, therapies, and activities, ensuring every resident receives appropriate support and engagement.
Journey offers Memory Care in six of our skilled nursing communities, including Glasgow Hills in West Virginia, Kirtland Woods in Ohio, Frederick Crossing in Maryland, Lyndon Crossing in Kentucky, and two locations in Georgia (Calhoun Crossing and Stone Mountain Run).
Our Care Team includes Certified Dementia Practitioners, Infection Control Preventionists, Therapists (Physical, Occupational, and Speech), Registered Dietitians, and other dementia-trained skilled nursing staff who provide expert, compassionate guidance every day.
